and group health plans to cover the cost of over-the-counter, at-home COVID-19 tests.

UHC COVID-19 AT HOME TESTING KIT INFORMATION
EFFECTIVE: JANUARY 15,2022
What can be Covered?
- Is effective through the end of the public health emergency (PHE), effective April 15, 2022
- Allows coverage of 8 testing kits per month per covered member that purchased at preferred OTC retailers
- Preferred OTC retailers: Walmart Pharmacy & Rite Aid (must go to the retail store)
- You can call the pharmacies below to see if there are testing kits available
- Must bring your UHC card that shows OptumRx for getting at home COVID-19 testing kits at NO COST

What is NOT Covered?
- SUBMIT purchase receipt for reimbursement at at myuhc.com or through a reimbursement form (pdf) for maximum reimbursement of $12 per test
- If testing kits purchased at any in-store or online retailer OTHER THAN the IN-STORE Walmart Pharmacy & Rite Aid
- If you do not see an OptumRX logo on your member ID card
- Please submit your claims to UHCG:

- Attached completed claim form
- Two-pack test kit will be reimbursed at $24 ($12 per each test kit)
- Each test is counted separately if multiple tests are sold in one package
- Unfortunately, any purchases BEFORE January 15,2022 will not be covered

- Each test is counted separately if multiple tests are sold in one package
- (i.e. if one package includes 2 tests, it counts as 2 testing kit; if one package has eight tests, it counts as eight tests and not one test package toward the quantity limit)
- Unfortunately, any purchases BEFORE January 15,2022 will not be covered
- Testing kit CANNOT be used for employment purposes
